Squamous Cell Carcinoma of the mediastinum Complicated with Pulmonary Hypertrophic Osteoarthropathy



Abstract
A 39 year old male patient who has been suffered from generalized bony and joint pain, particularly in both extremities since several years prior to admission.
On physical examination, both hands and feet were swollen. Mental depression and distended neck veins of right side were noted. The general appearance was that of the acromegaly. The pertinent tests were carried out to rule out acromegaly and pseudohypoparathyroidism. All such tests were inconclusive. Exploratory mediastinostomy showed huge fairly encapsulated hard mass occupying upper anterior mediastinum which was extended from lower pole of thyroid to base of heart involving pericardium and both visceral pleurae. The mass pressed over superior vena cava and right subclavian vein. The mass was bluntly dissected out through ill-defined cleavage plane. The specimen was measured about 10¡¿10¡¿6 cm in size.
The bony and joint pain disappeared almost completely after operation. The swelling of extremity and distended neck vein were relieved also by surgery.
The postoperative course was uneventful and postoperative irradiation was done.
The purpose of this report is to review the pulmonary hypertrophic osteoarthropathy with mediastinal tumor and to report primary mediastinal squamous cell carcinoma.
